2. Which activities use Form 224, and which use Form 224a?
DEA's registration page lists DEA Form 224 for a retail pharmacy, hospital/clinic, practitioner, teaching institution, or mid-level practitioner. The table in 21 CFR 1301.13 also places online pharmacy, central fill pharmacy, and emergency medical services agencies on Form 224. This page does not walk through screens or fields.

Effective May 11, 2022 (87 FR 21019), these applications and renewal submissions are filed online. DEA's forms index notes Form 224 is unavailable as a PDF. Track the submission and the receipt.
Manufacturers, distributors, researchers, importers, and exporters use Form 225 and Form 225a. Treatment programs use Form 363 and Form 363a. Online pharmacy modifications use Form 224c. Keep those packets off a Form 224 row. Fees are not restated here.

5. What happens when Form 224a submission records slip
A location can have no logged renewal submission, or a confirmation nobody can produce. Credentialing then treats the packet as absent. A satellite can lack a 224a receipt while the flagship file looks complete.
